Case Management at OTFC

When you become a client of OTFC, you become a member of our extended family and likewise we become a member of your service provision team. To ensure positive outcomes are reached for your child and your family, each of our Occupational Therapists (upon your request) will work with you and the rest of your child’s allied health and education team in a multi-disciplinary model. This type of case management can be but is not limited to:


  • Attendance at meetings
  • Letters, emails or phone calls to the rest of your child’s service provision team
  • Referrals to other services we believe your child needs
  • Recommendations to programs to help support and consolidate learning and promote growth
  • Planning for the future by helping you write a Therapy Plan
  • Engaging our Developmental Educator to integrate clinic based therapy with your home life
  • Key worker for your NDIS plan (all NDIS plan services are managed by Michelle)
